Job Type: Part-time

Description:

The Community Services Provider plays a vital role in providing direct care and support to individuals with developmental disabilities. This position involves implementing Habilitation Plans, which aim to promote independence, community integration, and exploration.

Key Responsibilities:

  • Provide supervision and direct care to ensure the safety and comfort of individuals receiving services.
  • Transport or accompany individuals on community outings, appointments, or recreation activities to promote community integration and exploration.
  • Develop and plan inclusive community activities that foster socialization and skill-building.
  • Establish and maintain positive relationships with area businesses to promote community engagement.
  • Assist individuals with daily or routine domestic or personal care tasks to promote independence.
  • Foster and promote skills leading to independent living, such as cooking, cleaning, and personal care.
  • Assist with the development of Habilitation Plans and follow these plans for training and assistance.
  • Provide training and assist individuals to develop, maintain, and/or improve desired individual goals, living skills, and personal needs.
  • Supervise individuals during program hours and community outings to ensure their health and safety.
  • Complete required documentation in conformance with rules, regulations, policies, and procedures, and billing practices.

SPECIAL REQUIREMENTS:

  • Possess good interpersonal, written, oral, and communication abilities, as well as effective time management and organizational skills.
  • Be computer literate with knowledge in agency operating systems and applicable software required for department needs.
  • Have regular access to the internet.
  • Demonstrate sound judgment in normal work and emergency situations.
  • Must physically be able to perform required certifications and lift 50 pounds.

Requirements:

  • High School Graduate/GED with some knowledge of or experience working with developmentally disabled individuals preferred.
  • Possess a valid driver's license acceptable to the agency's insurance carrier.

Salary: $16.00-$17.25/hr
