Job Summary

The Licensed Practical Nurse (LPN) will work collaboratively with the healthcare team to provide high-quality care to home health or hospice patients. The LPN will follow established plans of care, perform approved nursing procedures, and document patient interactions and medication administration.

Key Responsibilities
  • Provide home health or hospice services to patients through regular visits, following established plans of care.
  • Perform approved nursing procedures and treatments within the LPN scope of practice.
  • Document all interventions, patient responses, and medication administration in the medical record.
  • Assist in planning and evaluating nursing care for patients and their families.
  • Participate in performance improvement activities as requested.
Credentials and Education
  • Basic Cardiac Life Support (BCLS) certification.
  • Valid driver's license.
  • Pennsylvania LPN licensure.
  • High School Diploma or equivalent.
  • Graduate of an NLN-accredited licensed practical nursing program.
