Nurse Home Visitor Job Description

VNA Health Group is seeking a skilled Registered Nurse to join our team as a Nurse Home Visitor. This is a unique opportunity to make a meaningful difference in the lives of women and their families in Ocean County.

Job Summary:

The Nurse Home Visitor will provide comprehensive nursing services to women and their families eligible for the Nurse Family Partnership program. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Assess and evaluate the needs of women and their families, providing counseling and instruction to assist them in achieving targeted goals.
  • Carry a caseload of up to 25 clients, completing home visitations in accordance with the NFP model elements and NFP Visit-to-Visit guidelines.
  • Develop and maintain community relationships to support client referrals, precept and mentor new hires, and make visits with student nurses as needed.
  • Adhere to agency infection control and safety policies, as well as the organization's policy regarding absenteeism and appearance.
Qualifications:
  • Bachelor's degree in Nursing.
  • 2-3 years of work-related experience.
  • Strong computer skills.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and organizational skills.
  • Bilingual - Spanish speaking a plus.
