Job Summary:The Registered Nurse is responsible for providing high-quality, patient-centered care in the comfort of the patient's own home. This role requires strong clinical skills, excellent communication, and a passion for delivering exceptional patient care.
Key Responsibilities:- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide skilled nursing care, including wound care, infusion therapy, and catheter management.
- Educate patients and families on disease management, medication adherence, and self-care techniques.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress, identify areas for improvement, and adjust care plans as needed.
- Active RN license in the state of application.
- Valid CPR certification and driver's license.
- Preferred: Medicare skilled nursing experience, basic understanding of Oasis, and 1-year RN experience in a healthcare setting.
