About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Per Diem Hospice LPN. As a member of our interdisciplinary team, you will provide direct patient care services, ensuring that each individual receives the highest level of care and support.

Key Responsibilities

  • Administer medications, wound care, and vital sign monitoring to patients.
  • Assist in evaluating patient/family needs and developing the plan of care.
  • Communicate effectively with patients, families, and healthcare teams.
  • Prioritize tasks and manage time effectively to meet deadlines.

Requirements

  • Current LPN license in good standing.
  • Experience in hospice, home health, ICU, geriatric, or oncology settings preferred.
  • Ability to work autonomously and collaboratively as part of a team.
  • Familiarity with electronic medical records (EMRs) and other healthcare technology systems.
