Job Summary:

The Facilitated Telehealth Coordinator will serve as the in-person liaison between the patient and our virtual care team, facilitating communication between providers and their patients, and ensuring seamless delivery of care to patients in various settings.

Key Responsibilities:
  • Travel to patient homes or primary care offices to assist patients and/or caregivers with connecting to telehealth visits
  • Facilitate communication between providers and their patients, including assisting patients with the use of technology to conduct virtual visits
  • Assess environment regarding safety concerns or potential hazards
  • Communicate with the patient care team regarding patient questions and needs
  • Coordinate telehealth services and ensure seamless delivery of care to patients in various settings
  • Provide education and support to patients and caregivers on how to use telehealth technology effectively
  • Maintain accurate and up-to-date records of telehealth appointments and patient data
  • Collaborate with IT and technical support teams to resolve any telehealth system issues
  • Conduct training sessions to educate healthcare staff on telehealth best practices and workflows
  • Collaborate with cross-functional teams to identify technology needs and opportunities for improvement
  • Troubleshoot and resolve technology issues in a timely manner to minimize downtime
Requirements:
  • Comfortable working with the senior population and those with behavioral health conditions
  • Proven experience working in a healthcare setting or similar role
  • Knowledge of telehealth technology and software
  • Excellent organizational and time management skills
  • Strong attention to detail and ability to multitask
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaborate effectively with team members
  • Proactive and adaptable to change in a fast-paced environment
  • Possession of driver's license and car
  • Preferred Bilingual (Spanish-English)
  • Willingness to travel up to 100 miles round trip per day
Preferred Qualifications:
  • At least 1 year of experience directly related to the duties of the position
  • Passion for, and experience working with, patients with mental health conditions
  • Passion for, and experience working with, seniors
