Key Responsibilities:
  • Coordinate care between members, caregivers, and healthcare providers to ensure person-centered care.
  • Monitor member status, progress, and care plan goals, collaborating with members, caregivers, and providers to revise or update care plans as needed.
  • Provide resource support to members and care managers for local resources, such as employment, housing, and independent living services.
  • Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
  • Perform outreach to assess member needs and collaborate with resources.
  • Collect, document, and maintain member information and care management activities to ensure compliance with regulators.
  • Provide education to members and caregivers on disease processes, care options, referrals, and healthcare benefits.
  • Offer feedback to leadership on opportunities to improve care and quality delivery.
Requirements:
  • Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience.
  • RN - Registered Nurse - Compact Licensure required.
