Key Responsibilities- Provide personal care assistance, including toileting, showers, eating, and grooming, to maintain the participant's independence in conducting activities of daily living.
- Assist in the maintenance of physical activity, exercise, and walking to promote overall health and well-being.
- Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the center.
- Identify and report any signs or symptoms indicating physical, emotional, or behavioral change in participants, including changes in skin integrity, bowel and bladder status, food/fluid intake, or compliance with infection control and personal protection equipment standards.
- Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary, and assist transportation staff with loading and unloading of participants from vehicles.
- High school diploma or GED required.
- Active certified nurse assistant in the state where PACE Organization is located preferred.
- Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
