Job Title: Registered Nurse

VNA Health Group is seeking a skilled Registered Nurse to join our team in providing high-quality home health care services to patients in Manville, New Jersey.

Job Summary:

The Registered Nurse will be responsible for assessing and evaluating the needs of patients with multiple disease processes and comorbidities, developing and implementing individualized care plans, and providing education and strategies for patients and families.

Responsibilities:
  • Participate in a comprehensive clinical orientation to learn key procedures, the EMR system, and compassionate care practices.
  • Assess and evaluate patient needs, develop and implement individualized care plans, and document all changes in patient condition.
  • Provide education and strategies for patients and families to promote health and well-being.
  • Collaborate with other VNA clinicians and external providers to achieve best patient outcomes.
Qualifications:
  • RN with a Bachelor's degree in Nursing preferred.
  • 1+ year of supervised clinical experience as a registered nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills.
  • Excellent organizational and interpersonal skills.
  • Valid driver's license and reliable transportation.
