About the Role:
A skilled Physical Therapist is needed to join our team at Rehab America, where we provide top-notch care and support to patients recovering from surgery, injury, or medical crisis. As a Rehabilitation Therapist - Geriatric Care Specialist, you will be responsible for evaluating patients, creating personalized treatment plans, and delivering high-quality physical therapy services.

Key Responsibilities:
• Evaluate patients' functional abilities and create effective treatment plans
• Conduct diagnostic tests and assessments to identify areas for improvement
• Collaborate with healthcare teams to ensure seamless patient care
• Provide education and training to patients, families, and caregivers on proper techniques and strategies for rehabilitation
• Participate in quality improvement initiatives to enhance patient outcomes

Requirements:
• 1 year of experience in a hospital or skilled nursing facility setting
• Graduate of an accredited Physical Therapy Program
• Current Registered Physical Therapist License (Required)
• Current State Driver's License (Required)
