Job Summary

We are seeking a skilled PTA to join our Rehabilitation Team based out of South Portland, Maine. As a Home Health PTA, you will provide customized physical therapy to patients in their homes, collaborating with a multidisciplinary team to develop and implement effective care plans. Your expertise and compassion will enable patients to regain mobility, strength, and functionality, improving their overall quality of life.

Responsibilities

Your key responsibilities will include:

  • Providing one-on-one physical therapy to patients in their homes, focusing on exercises and hands-on therapy to manage pain and increase mobility.
  • Observing movements, conducting assessments, documenting progress, and modifying care plans as needed.
  • Educating patients and their families on established goals, exercises, and self-care strategies.
  • Collaborating with the home health team, including an RN, LPN, CNA, HHA, OT, and SLP, to ensure comprehensive care and coordinated efforts.
Requirements

To be successful in this role, you must possess:

  • A valid PTA License in the State of Maine.
  • A minimum of 1 year of PTA experience, preferably in home healthcare.
  • CPR certification and up-to-date vaccinations.
  • Strong clinical judgment, excellent interpersonal skills, dexterity, and compassion.
  • Knowledge of state and federal home health regulations.
