Job Summary
We are seeking an RN Care Manager to join our team at Tufts Medicine Integrated Network. As a RN Care Manager, you will be responsible for providing professional nursing care services in accordance with physician orders and performing care management duties to improve patient outcomes and create efficiencies. This role focuses on working as part of a multidisciplinary team to coordinate patient care and oversee the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education.

Key Responsibilities
* Provides professional nursing care services in accordance with physician orders
* Performs care management duties to improve patient outcomes and create efficiencies
* Works as part of a multidisciplinary team to coordinate patient care
* Oversees the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education

Requirements
* Massachusetts RN Licensure
* Obtain appropriate state board where services will be provided as a registered professional nurse
* OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) within three (3) months of hire
* Three (3) years of clinical experience in an acute care setting
* Valid driver's license in current state of residence
* Basic Life Support Certification AHA or certification providing classroom hands-on training for Healthcare Providers

Preferred Qualifications
* Bachelor's degree
* Five (5) years of experience in acute, subacute, or home care, palliative care and hospice experience a plus
* Case Management certification and experience
* Bilingual

Salary Information
The estimated salary for this role is between $85,000 and $110,000 per year, depending on experience. We offer a competitive benefits package, including health insurance, retirement savings plan, and paid time off.
