About the Role

Responsibilities
  • Interpretation and Transcription: Accurately interpret and transcribe physician orders in the pharmacy computer system, ensuring timely and efficient processing.
  • Pharmaceutical Dispensing: Compound and dispense pharmaceuticals with precision, adhering to established protocols and guidelines.
  • Medication Therapy Management: Monitor drug therapy regimens for contraindications, drug-drug interactions, and appropriateness of drug and dose, providing accurate and timely medication information to healthcare staff and patients.
  • Collaboration and Communication: Effectively interact with medical staff regarding drug therapy issue resolutions, ensuring seamless communication and collaboration.
  • Documentation and Record-Keeping: Document all clinical activities and interventions accurately and completely, maintaining up-to-date records and reports.
  • Formulary Management: Sustain the hospital formulary, minimizing non-formulary procurements, and utilizing therapeutic interchanges and other approved formulary protocols and guardrails.
  • Leadership and Supervision: Supervise and assist pharmacy technicians and other support staff effectively, prioritizing work and leading others to comply with facility medication turn-around time policy and procedure.
  • Training and Development: Support in the training and mentorship of new staff members as assigned, and may serve as a preceptor for pharmacy students and residents.
  • Quality Improvement and Patient Safety: Support and participate in the goals of the organization's quality improvement, risk management, and patient safety initiatives, including National Patient Safety Goals, Core Measures, and HCAHPS medication-related initiatives.
  • Professional Development: Maintain professional competency sufficient to meet current and future needs of the facility, and maintain appropriate controlled substance records.
Requirements
  • Education: B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
  • Licensure: Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
  • Certifications: BLS & ACLS certification (BLS required within 90 days of hire; ACLS required for attending Code Blue).
  • Compounding Requirements: Completion of the TSBP educational requirements in the field of compounding sterile preparations OR must be capable of completing requirements within 90 days of hire date.
Preferred Qualifications
  • Doctor of Pharmacy: Doctor of Pharmacy from an ACPE-accredited college of pharmacy.
  • Experience: One year of inpatient hospital pharmacy experience.
