Role Summary:

This full-time position involves overseeing medical case management staff, ensuring patient issues are resolved, and maintaining clinical supervision logs. You will also perform quality assurance activities and facilitate communication among care teams to improve patient outcomes.

Key Responsibilities:
  • Provide day-to-day operation oversight of the medical case management staff.
  • Resolve patient concerns and ensure smooth operation of medical case management services.
  • Create and maintain up-to-date clinical supervision logs.
  • Perform quality assurance and quality improvement activities.
  • Facilitate ongoing care team communication to improve patient care.
Requirements:
  • An advanced degree in a related field (health, nursing, mental health, or public health) with 3 years of full-time experience required.
  • Two years of supervisory experience preferred.
  • Experience working with target populations (Serious Mental Illness, substance use disorders, immigrants, LGBT, HIV/AIDS) strongly preferred.
  • UAS-NY certification within 1 month of hire required.
  • A commitment to Apicha's mission and values.
  • Excellent MS Office skills.
  • Culturally competent representation of Apicha CHC.
  • Strong customer service skills.
  • Excellent verbal and written communication skills.
Compensation and Benefits:

Competitive salary: $80,000 - $110,000 per year, depending on experience, plus a comprehensive benefits package including paid holidays, vacation and sick leave, health insurance coverage, flexible spending account, life insurance, and an employee assistant program.
