We are seeking a skilled Home Health Physical Therapist to join our team in Denver, Colorado.

About the Role

This is an exciting opportunity to work with a diverse range of patients and contribute to their recovery and well-being. As a Home Health Physical Therapist, you will be responsible for delivering expert physical therapy services and interventions to pediatric patients dealing with genetic, neurological, cognitive, and orthopedic disorders within natural learning environments, including homes and community settings.

You will conduct comprehensive evaluations of assigned patients and develop, implement, and adjust treatment plans to align with individual patient needs. This may include providing teaching and counseling services to both patients and their families, fostering a supportive and informative therapeutic process.

In this role, you will collaborate seamlessly with a multidisciplinary team, actively participating in the planning and evaluation of team goals for each patient. You will also ensure precise and timely documentation, adhering to the standards set by the department and regulatory agencies.

Requirements
  • Bachelor's, Master's, or Doctorates degree from an accredited therapy program
  • Active state licensure as required
  • Current Colorado Physical Therapy License is required
  • CPR Certification
  • Valid driver's license and insurance
