CarePartners is seeking a highly skilled Post-Acute Transition Specialist to join our team. As a key member of our care management team, you will play a critical role in ensuring seamless transitions for patients across the continuum of care.
Key Responsibilities- Establish and maintain relationships with internal and external referral sources, including hospitals, skilled nursing facilities, and physician practices.
- Provide education and promote collaboration to ensure optimal patient placement and care.
- Work closely with post-acute providers, SNFs, and physician practices to ensure a responsive and smooth referral and admission process.
- Collaborate with health system and community colleagues to promote quality-driven, best practice care across the continuum.
- Maximize positive financial outcomes for designated patient populations by facilitating appropriate post-acute placement and participating in Goals of Care/Advanced Care Planning dialogue.
- Bachelor of Science in Nursing or related field.
- Minimum of two years recent experience in a health-related field.
- Preferred experience: eight years recent clinical experience in a relevant practice area.
- Speech Pathologist, Registered Nurse, Occupational Therapist, or Licensed Physical Therapist certification required.
