Responsibilities
- Provide primary care and mental health services to patients in various settings, including nursing homes and assisted living facilities.
- Work closely with the OnSite team, nursing home staff, residents, and families to ensure comprehensive care.
- Assess patients' needs and develop treatment plans to address their medical and mental health concerns.
- Provide education on best practices and integrate various interventions to promote patient well-being.
- Master of Science in Nursing with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification.
- Active License in South Carolina.
- DEA registration.
- Excellent communication and documentation abilities.
- Strong patient assessment skills.
- Strong, ambitious, patient-centric work ethic.
