Job Summary

We are seeking a skilled Physical Therapist to join our Homecare team at Intermountain Healthcare. As a Physical Therapist, you will provide high-quality physical therapy services to patients in their homes, working closely with other healthcare professionals to ensure comprehensive care.

Key Responsibilities
  • Provide quality physical therapy services to patients in their homes, adhering to state licensure, laws, professional standards, and ethics.
  • Coordinate care with all disciplines involved, ensuring seamless patient care.
  • Develop and implement individualized care plans, prioritizing patient needs and goals.
  • Communicate effectively with patients, families, and healthcare teams to ensure timely and thorough documentation of patient records.
  • Manage time, supplies, and equipment efficiently, meeting established productivity standards.
Requirements
  • Current Physical Therapy License in the state of practice.
  • Basic life support certification (BLS) for healthcare providers.
  • Access to the Internet or a phone line for transferring patient files.
  • Reliable transportation and an acceptable driving record.
Preferred Qualifications
  • Demonstrated effective communication skills.
  • Basic computer skills.
Work Environment

This is a part-time, non-exempt position with a variable schedule, including rotating Saturdays. The hourly rate is $63.59, and benefits are eligible.
