Job Summary

The Clinical Team Leader (CTL) is a crucial role at Enhabit Home Health & Hospice, responsible for ensuring the coordination of home health services to clients is delivered according to acceptable standards of practice. Responsibilities

As a CTL, your responsibilities will include:
  • • Assisting the Branch Director in managing patient care and staff
    • Overseeing the electronic review and approval of patient information
    • Providing oversight of patient care and supporting staff development
    • Collaborating with physicians, referral sources, patients, caregivers, and employees to ensure effective communication and care delivery
    • Participating in an on-call rotation and making patient visits as needed

Qualifications

To be successful in this role, you will need:
  • • A graduate degree from an accredited school of professional nursing and licensure in the state of employment
    • At least two years of nursing experience, with one year in home health
    • Strong understanding of federal, state, and local laws governing home health agencies
    • Skilled competency with technology, including general office equipment and EMR platforms
    • Valid state driver's license and automobile liability insurance
