Job Summary

The Home Health Physical Therapist restores patients' function, alleviates pain, and prevents disabilities by planning and administering medically prescribed therapy services in the home setting. You will demonstrate knowledge and skills necessary to provide care for the population served by Avera@Home.

Responsibilities
  • Plan and administer medically prescribed therapy services in the home setting
  • Demonstrate knowledge and skills necessary to provide care for the population served by Avera@Home
  • Work collaboratively with other healthcare professionals to achieve optimal patient outcomes
Requirements
  • Graduate from an accredited physical therapy program preferred
  • One or more years experience in-home health physical therapy preferred
  • Licensed by the State of South Dakota as a Physical Therapist required
  • Licensed as a Physical Therapist in the states of Nebraska, Iowa, Minnesota, or North Dakota may be required depending on the service area
