Position Summary:
Under the direct supervision of the Primary Care Physician and/or Medical Director, the Nurse Practitioner utilizes advanced clinical skills and knowledge to assess and manage the healthcare needs of chronically ill geriatric participants in the LIFE program. The role involves daily operational collaboration with the Clinic Nurse Manager.
Key Responsibilities:
- Operate within the defined scope of practice as per state regulations.
- Adhere to physician oversight requirements and established parameters.
- Engage as a member of the Interdisciplinary Team (IDT) and collaborate with other required IDT members.
- Work closely with the Primary Care Physician/Medical Director to ensure optimal clinical outcomes in line with LIFE-NWPA's Quality Assurance and Performance Improvement (QAPI) program.
- Coordinate with the Clinic Nurse Manager on operational and documentation matters, including scheduling and charting.
- Conduct health histories, physical examinations, and patient education. Order, interpret, and follow up on laboratory tests and results.
- Initiate emergency protocols as necessary and recognize situations requiring immediate referral.
- Assess, initiate, and coordinate patient care daily under the supervision of the attending physician.
- Serve as a liaison among physicians, nurses, and other healthcare professionals, providing education and support.
- Participate in the coordination of 24-hour care delivery.
- Attend staff meetings and contribute to participant care planning.
- Engage in and support Quality Improvement initiatives.
- Perform additional duties as assigned.
Required Skills and Abilities:
- Ability to interact tactfully and patiently with personnel, participants, families, visitors, and the public.
- Capacity to make independent decisions when necessary.
- Cognitive ability to process and perform tasks ensuring participant safety.
- Commitment to ongoing education and professional development.
- Willingness to travel for training and in-service programs.
- Aptitude for learning, teaching, and conducting research; strong analytical problem-solving skills.
- Organizational skills to manage work and follow through on tasks.
- Current CPR and First Aid Certification.
- Exceptional written and verbal communication skills.
Education and Experience Requirements:
- Must hold a valid Certified Nurse Practitioner license in the State of Pennsylvania.
- Minimum of one year of experience working with the elderly or willingness to undergo extensive training.
Additional Requirements:
- Valid State driver's license with a clean driving record.
- Successful completion of background checks, physical examinations, drug screenings, and TB screenings.
Physical Demands:
- Ability to move intermittently throughout the workday.
- Complete comprehensive orientation to the LIFE/PACE model of care, which may involve overnight travel.
- Subject to lifting, carrying, and supporting participants.
- Must be capable of driving a motor vehicle.
