Job Summary

We are seeking a highly skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will be responsible for providing comprehensive speech and language evaluations, treatment, and related follow-up services to patients in their homes.

Key Responsibilities

  • Conduct thorough speech and language evaluations to identify patient needs and develop personalized treatment plans.
  • Provide speech therapy services, including the selection and use of assistive equipment, to patients in their homes.
  • Collaborate with patients, families, and other healthcare professionals to coordinate care and ensure seamless transitions.
  • Document patient assessments, treatment plans, and progress in a timely and accurate manner.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improvement.

Requirements

  • Master's degree in Speech Language Pathology from an accredited program.
  • Current licensure as a Speech Language Pathologist in the state of practice.
  • 1 year of experience in speech therapy, preferably in a home health setting.
  • Knowledge of speech and language disorders, as well as treatment modalities and techniques.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
