Position Overview
Elderwood is seeking a dedicated and professional Physical Therapist to contribute positively to the health and wellness of our residents and their families.
Role Summary:
The Physical Therapy team plays a crucial role in promoting the health and recovery of individuals by delivering comprehensive and high-quality therapy services. This role entails administering specialized physical therapy treatments while ensuring the utmost quality of care is upheld at all times. A valid state-specific Physical Therapy license is mandatory.

Key Responsibilities:
- Initiate and execute skilled physical therapy treatments as per physician's directives; maintain necessary communication with the physician.
- Document evaluations, progress notes, and discharge summaries for patients in the physical therapy program.
- Collaborate with other healthcare professionals to address the specific needs of individuals to enhance rehabilitation outcomes.
- Participate in meetings and provide input during the absence of the Director of Physical Therapy.
- Maintain accurate statistical records of patient treatments.
- Assist in training programs as requested.
- Update and record individual care plans in the Inter-disciplinary Kardex.
- Request necessary supplies for treatment from the Director of Physical Therapy.
- Assume the responsibilities of the Director of Physical Therapy during their absence, including supervising Physical Therapy Aides and ensuring all documentation is completed.
Qualifications:
- Graduation from an accredited physical therapy program is required.
- A valid state license to practice physical therapy is essential.
- Preferred: Two years of experience in physical therapy.
- Ability to lead and direct others is necessary.
- This position requires regular interaction with residents, coworkers, visitors, and supervisors. Proficiency in reading, writing, speaking, and understanding the English language at an intermediate or advanced level is required to ensure effective communication and a safe work environment.
