As a Home Health Physical Therapist, you will be responsible for assessing and treating patients with various physical and functional limitations, promoting independence and improving their quality of life.

Responsibilities:
  • Conduct thorough assessments of patients' physical and functional abilities
  • Develop and implement individualized care plans to address patients' needs and goals
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to improve patients' mobility and function
  • Educate patients and their families on proper care and management techniques
  • Document patient progress and communicate with healthcare teams to ensure seamless care
Requirements:
  • Current NJ Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
