Key Responsibilities
  • Provide direct patient care through routine and urgent in-home visits, under the supervision of an authorized healthcare practitioner.
  • Facilitate virtual visits to support care delivery, working closely with a remote clinical practitioner.
  • Perform a range of clinical procedures, including blood draws, laboratory specimen collection, medication administration, and wound care, while adhering to clinical guidelines and best practices.
  • Coordinate patient care by implementing care plans and communicating effectively with patients, caregivers, and clinical partners.
  • Establish trusting relationships with patients, their families, and caregivers, providing personalized care that addresses their unique needs and preferences.
  • Maintain accurate records, adhere to regulatory requirements, and participate in ongoing education and training to enhance your skills and knowledge.
