Job Summary:
We are seeking a skilled LPN Supervisor to join our team at Cooperstown Center. As a key member of our nursing staff, you will be responsible for overseeing the daily operations of our facility and ensuring the highest level of care for our residents.
Key Responsibilities:
- Conduct regular facility rounds to monitor resident care and identify areas for improvement
- Supervise and coordinate the work of nursing aides to ensure efficient and effective care delivery
- Intervene in resident conflicts and behavioral issues, providing support and guidance as needed
- Oversee medication administration and ensure accurate documentation and follow-up
- Facilitate communication among healthcare team members to ensure timely and effective care
- Assist with late admissions and re-admissions, including paperwork and medication changes
- Report to the Director of Nursing (DON) and collaborate with other department leaders to achieve facility goals
Requirements:
- Minimum 5 years of recent clinical nursing experience, with at least 1 year in Long Term Care or sub-acute settings
- Valid state LPN license and good standing with the State Registry
- Strong leadership and communication skills, with ability to work effectively in a team environment
