About the Role

We are seeking a skilled Mental Health Therapist to join our team at Places for People (MO). As a key member of our team, you will provide disease management and skill promotion services to individuals with mental illness, developmental disabilities, and/or substance use disorder.

Key Responsibilities
  • Provide person-centered services to help individuals achieve their goals
  • Conduct regular visits with individuals in the community and elsewhere, as outlined in their treatment plan
  • Write, implement, and monitor Individualized Treatment Plans with individuals within agency guidelines
  • Coordinate with healthcare providers to ensure effective coordination and continuity of care
  • Facilitate self-sufficiency and maximize independence in the community
  • Link and refer individuals to community resources as needed
  • Provide crisis intervention and resolution for individuals as necessary
  • Maintain confidentiality of individuals as outlined by agency policy
Requirements
  • Masters level social work or counseling in progress and at least 1 year of relevant experience providing services to individuals with mental illness, developmental disabilities, and/or substance use disorder
  • CPR, First Aid, and Medication Administration certifications required
  • Chauffeur's License, personal vehicle, and valid auto insurance required
