Habilitation Specialist

The Resource Center is seeking a skilled Habilitation Specialist to join our team. As a Habilitation Specialist, you will play a critical role in supporting individuals with disabilities to achieve their life goals and live fulfilling lives.

The Habilitation Specialist will work closely with individuals, their families, and support teams to develop and implement comprehensive habilitation plans. This will involve identifying and addressing the unique needs and goals of each individual, and providing support and guidance to help them achieve their desired outcomes.

• Develop and implement individualized habilitation plans that align with the person's life goals and values
• Provide support and guidance to individuals and their families to help them navigate the habilitation process
• Collaborate with support teams to ensure seamless delivery of habilitation services
• Monitor and evaluate the effectiveness of habilitation plans and make adjustments as needed
• Ensure compliance with regulatory requirements and agency policies

• Bachelor's degree in Human Services or related field
• Minimum of one year of experience in a program serving individuals with disabilities
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver's license and reliable transportation
