Job Description

Summary/Objective:

The Foster Care Case Manager, under the guidance of the Casework Coordinator, is responsible for delivering comprehensive foster care services to a designated caseload. This role involves collaborating with the youth and their families as part of a treatment team to achieve established permanency planning objectives. The Case Manager offers support, mentorship, and guidance to youth in foster care, their biological families, and certified foster parents. In partnership with service providers, family members, and clinical staff, the Case Manager conducts strength-based assessments and formulates personalized treatment plans. Additionally, the Case Manager connects youth and families with community-based support services and resources as necessary.

Core Responsibilities:

  1. Conducts strength-based assessments of family members.
  2. Maintains communication with service providers regarding participants' attendance and progress toward their goals.
  3. Coordinates visitations for each youth with identified resources, providing supervision to evaluate family progress.
  4. Engages with each youth, family member, and foster parent according to established agreements and regulatory requirements.
  5. Collaborates with the Treatment Team, including the child and family, to create Individualized Treatment Plans and identifies goals for each family member.
  6. Offers supportive counseling to youth, biological parents, and foster parents.
  7. Participates in regular Treatment Team Meetings.
  8. Works with community organizations for long-term planning and support, including the recruitment and retention of foster homes.
  9. Attends Family Court hearings, assists in preparing necessary documentation, and provides testimony as needed.
  10. Acts as a liaison for CHJC with various private and public service providers and resource agencies.
  11. Completes all mandated reports and forms in a timely manner.
  12. Inspects foster homes for health and safety compliance, addressing any concerns that arise.
  13. Ensures the safety and well-being of each youth in their birth home, foster home, and community, adjusting safety plans as required.
  14. Documents all casework activities thoroughly, including face-to-face contacts, phone calls, case conferences, court appearances, and home inspections.
  15. Maintains organized case files in accordance with relevant policies.
  16. Collaborates with assigned Care Coordinators to ensure the provision of necessary services, including health care coordination.
  17. Submits referrals for youth and biological parents for services such as mental health counseling and family support.
  18. Oversees school enrollment and monitors educational programming, fostering relationships with local schools to support each child's academic progress.
  19. Maintains case records and prepares ongoing reports for all children and families receiving services.
  20. Facilitates Life Skills Instruction as needed.
  21. Participates in the on-call rotation.
  22. Performs additional duties as assigned.

Work Environment:

This position involves both office work and fieldwork, requiring flexibility in scheduling to meet program needs. Remote work may be authorized.

Physical Demands:

The role requires various physical activities, including standing, walking, sitting for extended periods, and engaging in problem-solving tasks.

Position Type/Expected Hours of Work:

This is a full-time position, requiring 37.5 hours of work per week.

Travel:

Travel is a necessary component of this role.

Required Education and Experience:

A Bachelor's Degree in Social Work or a related Human Services field is required, along with two years of experience working with at-risk youth and families. Experience in child welfare, mental health, and casework is preferred.

Additional Eligibility Qualifications:

Successful completion of pre-employment requirements mandated by relevant state departments, including health assessments and background checks, is necessary. A valid driver's license with a good driving record is also required.
