LPN Ambulatory Chronic Care Specialist

We are seeking a skilled LPN to join our team in Ambulatory Chronic Care. As a key member of our healthcare team, you will provide high-quality patient care and support to patients of all ages.
Responsibilities- Provide patient care and complete required documentation for patients of all ages.
- Administer medications and monitor patients for adverse reactions.
- Provide education and information to patients and family members.
- Maintain equipment and supplies.
- Participate in quality initiatives and process improvements.
- Assist with departmental orientation and training of new employees.
- Maintain accurate and up-to-date documentation and files.
- Graduate of an approved school of Practical Nursing.
- Current Ohio License as an LPN M-IV or LPN Meds certification.
- Basic Life Support (BLS) through American Heart Association (AHA).
- Two years of patient care experience.
This role requires full body motion to move and lift patients, manual finger dexterity, and good hand-eye coordination. It also involves extensive standing, walking, and occasional lifting. Requires corrected vision and hearing to normal range.
Personal Protective EquipmentFollows standard precautions using personal protective equipment as required.
