At UW Health, we are seeking a Per Diem Registered Nurse (RN) Transitional Care Case Manager to bridge inpatient to ambulatory care for medical, surgical, and trauma populations.

As a key member of our interdisciplinary team, you will coordinate with healthcare professionals to ensure remarkable healthcare and empower patients during post-acute hospitalization recovery.

Our Per Diem RN Transitional Care Case Manager will have access to a variety of schedules and shifts, allowing for flexibility and work-life balance. You will also have access to UW Health's Wellness Options at Work, which support employee and family well-being.

Key responsibilities include:

  • Bridging inpatient to ambulatory care for medical, surgical, and trauma populations
  • Coordinating with interdisciplinary teams to ensure remarkable healthcare
  • Using clinical expertise to empower patients during post-acute hospitalization recovery

Requirements include:

  • Minimum - Bachelor's degree in Nursing
  • Preferred - Master's degree in Nursing or Health Care related field
  • Minimum - Three (3) years relevant clinical nursing experience
  • Preferred - Surgical, trauma, and intensive care experience
  • Minimum - RN licensed in the state of Wisconsin
  • Preferred - ACMA certification as a case manager
