A career opportunity has arisen for a Clinical Care Specialist to join our team in Idaho and Montana.

At Optum, we are committed to providing high-quality healthcare services to our patients. As a member of our Home and Community Care team, you will play a vital role in delivering comprehensive care to patients in the comfort of their own homes.

Your primary responsibility will be to conduct annual clinical assessments to identify and address health risks. You will work closely with our integrated medical teams to coordinate follow-up care and improve patient outcomes.

You will spend approximately 85% of your time traveling across the states of Idaho and Montana, conducting home visits and assessing patient needs. All travel expenses will be covered, and you will receive a competitive base salary, plus multiple incentives.

The ideal candidate will possess an active NP license and ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification. Additionally, you should have at least one year of clinical experience in a similar setting.

Key Responsibilities:

  • Conduct in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate
  • Identify diagnoses to inform care management and active medical management in furtherance of treatment
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
  • Communicate findings in your assessment to inform members' PCPs of potential gaps in care
  • Identify urgent and emergent situations and intervene appropriately
  • Educate members on topics such as disease process, medication, and compliance
