About the Role
As a skilled Home Health and Palliative Care Nurse with Knute Nelson, you will have the opportunity to provide compassionate care to patients in their own homes. This role requires strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality patient care.

Key Responsibilities:
  • Provide case management for hospice clients, including admission and ongoing assessments, charting, and developing individualized plans of care.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each patient.
  • Deliver palliative care and pain management services, utilizing evidence-based practices to optimize patient outcomes.
  • Conduct regular assessments to identify changing patient needs and make adjustments to care plans as necessary.

Requirements:
  • Current Registered Nurse licensure in Minnesota or North Dakota.
  • Minimum 2 years of experience in home health or hospice nursing.
  • CPR certification within 6 months of hire.
  • Adequate computer skills and ability to navigate electronic medical records.
  • Valid driver's license and use of an insured personal vehicle.

Estimated Salary: $83,000-$110,000 per year
