Job Title: Registered Nurse - Home Health Visits

BAYADA Home Health Care is seeking a skilled Registered Nurse to provide high-quality care to clients in their homes.

Job Description:
  • Provide comprehensive nursing care to clients in their homes, including medication administration, wound care, and treatments.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with clients, families, and healthcare teams to ensure seamless care coordination.
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care plans.
  • Document all client interactions, assessments, and care plans in a timely and accurate manner.
Responsibilities:
  • Follow established care plans and protocols to ensure high-quality care.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
  • Monitor client conditions and report any changes to the Clinical or Client Services Manager.
  • Perform client assessments and evaluations to identify areas for improvement.
  • Document all client interactions, assessments, and care plans in a timely and accurate manner.
Qualifications:
  • A current license as a Registered Nurse in Pennsylvania.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience is strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
