As a Licensed Practical Nurse, you will work closely with physicians, RNs, CNAs, and aides to deliver high-quality patient care. Your responsibilities will include taking patient vitals, conducting assessments, documenting progress, and reporting changes to the RN supervisor. You will also assist RNs in carrying out patient plans of care and teaching patients self-care techniques. Additionally, you will educate families on patient plans of care, medication, and home safety, and collaborate with the interdisciplinary team to coordinate patient care and plan for discharge.

To be successful in this role, you will need to have a graduate degree from an accredited practical nursing program and an active LPN license in New Jersey. You will also need a minimum of 1200 hours of LPN experience or completion of our office's preceptor program. CPR certification and knowledge of state and federal home health regulations are also required. Strong clinical proficiency in nursing, excellent communication skills, and compassion are essential for this role.
