Job Summary
We are seeking a Patient Outcomes Specialist - Renal Care Coordination to join our team This role will play a critical part in improving patient outcomes by coordinating care across multiple settings, promoting home dialysis modalities, and educating patients on self-management.
Responsibilities
- Conduct comprehensive assessments, identifying gaps in care and barriers to good health, managing a panel of approximately 150 assessed patients.
- Create and implement care plans tailored to individual patient needs, collaborating with healthcare teams to address identified issues.
- Care coordination involves advocating for patients, facilitating transitions between care settings, and minimizing avoidable readmissions.
- Assess patient knowledge of renal conditions and provide education and support for self-management.
- Provide ongoing reassessment and follow-up to continually improve patient outcomes.
- Clinical oversight includes delegating tasks to non-licensed support teams and social workers.
Requirements
- At least 1 year of experience in case management or care management, preferably coordinating care across multiple settings.
- 2+ years of healthcare-related experience.
- Current, unrestricted compact Registered Nurse license.
- Maintain current BLS certification from a licensed AHA or American Red Cross training facility.
- Demonstrate core values aligned with a patient-centered approach to care.
- Proactively act as a patient advocate and respond with resolve.
- Knowledge and experience empowering patients in self-management and shared decision-making.
- Effective written and verbal communication skills, demonstrating respect and cultural awareness during interactions.
- Strong analytical and critical thinking skills, community engagement, and facilitation skills.
Preferred Qualifications
- Bachelor's degree in nursing.
- Demonstrates empathy, enthusiasm, a great sense of humor, and a strong work ethic.
- Experience working with vulnerable patient populations (ESRD, geriatrics, minorities, low-income, uninsured).
- Ability to establish rapport with patients and families through inquiry and active listening.
- Familiarity with electronic medical records.
- Community outreach experience preferred.
Salary Information
The estimated annual salary for this position is $73,000-$90,000 based on national averages and industry standards.
