We are seeking a skilled Hospice Nurse Practitioner to join our team at Heartland Hospice. In this role, you will play a vital part in providing high-quality patient care and support to individuals and their families.
Overview:The Hospice Nurse Practitioner will be responsible for conducting Face-to-Face encounters to gather clinical information and collaborating with the hospice physician to ensure accurate patient eligibility for hospice services. This position requires a Master's degree in Nursing and relevant certification, ensuring quality care and support in our communities.

Responsibilities:- Conduct physical assessments for Face-to-Face encounter visits within established regulatory time frames.
- Provide clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists, to the recertifying physician.
- Make recommendations to the Interdisciplinary Group (IDG) based on F2F assessment findings.
- Notify the hospice agency when unable to complete a scheduled F2F visit.
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- Collaborative agreement in place.
- A competitive salary range: $85,000 - $110,000 annually.
