Job Title: Physical Therapist - Home Health

About the Role:
Adoration Home Health and Hospice is seeking a skilled Physical Therapist to join our team in Shelbyville. As a Physical Therapist, you will provide high-quality care to our patients in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans that promote patient independence and function.

Responsibilities:

  • Conduct thorough physical therapy evaluations and assessments to identify patient needs and goals
  • Develop and implement individualized treatment plans to address patient needs and goals
  • Provide skilled physical therapy interventions, including exercises, modalities, and education
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Doctor of Physical Therapy degree from an accredited program
  • Current licensure as a Physical Therapist in the state of operation
  • Minimum one year of experience as a Physical Therapist in a home health or skilled nursing setting
