Job Summary:

As an RN Case Manager, you'll be responsible for assessing, developing, and implementing discharge plans for patients. You'll work closely with our interdisciplinary team to identify and address any barriers to care, ensuring our patients receive the support they need to thrive.

Key Responsibilities:

  • Ensure compliance with Condition of Participation (CoP) discharge planning
  • Identify and escalate gaps/barriers related to inpatient and observation length of stay
  • Assess patients at high risk for readmission and develop plans to prevent readmission
  • Collaborate with the multidisciplinary team to develop and implement discharge plans
  • Participate in quality improvement initiatives to improve patient outcomes

Requirements:

  • Graduate of accredited school of nursing or education equivalency for licensing
  • Three years' acute care experience
