Job Title: RN Case Manager

The RN Case Manager is a vital member of our team at Jameco Home Health Agency Inc., responsible for ensuring that patient care is coordinated and managed effectively.

Key Responsibilities:
  • Coordinate patient care with healthcare professionals, including skilled nurses, MSW, and CHHA.
  • Assign supervisory visits for LVNs and CHHAs, and ensure timely submission of Oasis/visit notes.
  • Coordinate lab, pharmacy, and medication orders, as well as staff and MD communication.
  • Track and obtain authorization for HMO, and coordinate DME orders and supplies.
  • Ensure a CHHA plan is present if applicable, and inform visiting staff of patient transfers.
Requirements:
  • One (1) year experience in home health and/or case management.
  • Must be able to pass a background check.
  • Eligible to work in the US.
  • Basic Microsoft Office Excel, Word, and Anti-Virus skills.
  • Strong attention to detail, note-taking, and organizational skills.
  • Positive attitude, team success, and willingness to study, train, and learn skills.
  • On-call company cell phone, after hours (rotational).
Preferred Qualifications:
  • Bilingual or English proficiency.
  • Comfortable communicating quickly and efficiently via email and text.
