Discharge Planning Registered Nurse

We are seeking a skilled Discharge Planning Registered Nurse to join our team at Methodist Hospital Northeast. As a key member of our healthcare team, you will play a vital role in ensuring the smooth transition of patients from our hospital to their next level of care.
Key Responsibilities- Facilitate patient throughput by identifying pending and confirmed discharges, and communicating with the Patient Placement Center for direction.
- Assist in managing the discharge process, including reviewing discharge paperwork and communicating with physicians and Case Management as needed.
- Identify barriers related to timely discharge and communicate with the healthcare team to resolve issues.
- Review with Charge Nurses anticipated discharges for the following day to assist with downgrading patients to a lower level of care if appropriate.
- Communicate with physicians and Case Management for anticipated discharge needs prior to the day of discharge.
- Assist in identifying core measure patients and ensure criteria have been met and completed prior to discharge.
- Graduate of an accredited diploma, associates, or baccalaureate degree registered nursing program.
- Minimum 5 years of clinical nurse experience.
- Current Registered Nurse License in the State of Texas or Multi-State Compact License.
- Current American Heart Association BLS Certification.
- Meditech knowledge helpful.
- Excellent communication skills.
