As a Nurse Practitioner in our PACE program, you will play a critical role in delivering high-quality, patient-centered care to our participants.

Key Responsibilities
  • Primary Medical Coverage: Provide comprehensive medical care to our participants, including initial assessments, semi-annual reassessments, and unscheduled evaluations.
  • Collaboration with Interdisciplinary Team: Work closely with our interdisciplinary team to develop and implement participant plans of care, ensuring seamless coordination of services.
  • Medical Management: Manage participants' chronic illnesses and conditions, including prescribing medications, therapies, and other treatments as needed.
  • Preventive Health Care: Provide preventive health care and maintenance services, including immunizations, screenings, and monitoring of pertinent indicators.
  • Education and Training: Educate participants and families on self-care and specific medical conditions, promoting health literacy and empowerment.
  • Quality Assurance: Participate in quality assurance initiatives, including peer review and continuous quality improvement activities.
Requirements
  • Education: Graduate of an accredited school of nursing.
  • Experience: Minimum of 1 year working with the frail or elderly.
  • Licensure/Certifications: RN License – New Jersey; APN License – New Jersey Board of Nursing; Board Certification as a Nurse Practitioner with prescriptive authority.
