About the JobWe are currently seeking an experienced Home Health Physical Therapist to join our team of talented healthcare professionals. Job SummaryThe successful candidate will be responsible for assessing patient needs, developing and implementing individualized treatment plans, and providing hands-on therapy sessions to promote optimal physical function and overall well-being. Responsibilities:- Conduct thorough assessments of patients' physical capabilities and develop personalized treatment plans to address specific goals and objectives.
- Deliver high-quality physical therapy services, including exercise programs, modalities, and manual therapies, to promote maximum mobility and independence.
- Collaborate with other healthcare professionals, such as nurses, occupational therapists, and speech-language pathologists, to ensure comprehensive patient care and achieve optimal results.

Requirements:- Current professional license as required by the state.
