About the Role

We are seeking a skilled Nurse Practitioner to join our Clinical team at EasyHealth. As a key member of our team, you will conduct comprehensive health risk assessments for Medicare, Medicaid, and Commercial Marketplace members/patients of primary care physicians, medical groups, and the health plans we serve.

The ideal candidate will have 2 years of hands-on experience as a Nurse Practitioner, be registered with Medicare and Medicaid, and hold a current, unrestricted license in the state of Illinois. Additionally, they must have a license/certification as an APRN, Certified Nurse Practitioner, and possess knowledge of the HCC model, HEDIS, and STAR quality measures.

Responsibilities will include:

  • Completing detailed evaluations of the medical and social assessment of beneficiary members
  • Identifying and referring members/patients for routine, urgent, and emergent case management
  • Making recommendations for members' follow-up with their primary care physician
  • Not required to prescribe or alter the member's current treatment plans
  • Completing comprehensive assessments face-to-face in-home settings or via telehealth visits
