Job Title/Position: Registered Nurse (RN)

Reports To: Clinical Supervisor/Nursing Supervisor

JOB DESCRIPTION SUMMARY

The registered nurse plays a vital role in providing high-quality patient care in the comfort of their own homes. With a focus on community health education and experience, this professional nurse builds on the resources of the community to plan and direct services that meet the unique needs of individuals and families.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Patient Care

1. Conducts a comprehensive assessment of patient needs to determine the best course of care.

2. Develops and implements a personalized care plan that addresses the patient's physical, emotional, and social needs.

3. Provides skilled nursing care, including administering medications and treatments as prescribed by the physician.

4. Educates patients and their families on health management and disease prevention.

5. Collaborates with the patient's physician and other healthcare professionals to ensure seamless care coordination.

6. Identifies and addresses any barriers to care, ensuring that patients receive the support they need to achieve optimal health outcomes.

7. Maintains accurate and detailed records of patient care, including progress notes and treatment plans.

8. Participates in on-call duties as defined by the on-call policy, providing 24/7 support to patients and families.

Communication

1. Effectively communicates with patients, families, and healthcare professionals to ensure that care is coordinated and patient needs are met.

2. Prepares and updates clinical notes, ensuring that the patient's physician is informed of any changes in their condition.

3. Collaborates with community health resources to ensure that patients receive comprehensive care.

Additional Duties

1. Participates in quality improvement initiatives to enhance patient care and outcomes.

2. Stays up-to-date with the latest clinical guidelines and best practices in home health care.

3. Collaborates with the home health care team to ensure that patients receive seamless care coordination.

POSITION QUALIFICATIONS

1. Registered nurse with current licensure to practice professional nursing in the state.

2. Graduate of a National League for Nursing accredited school of nursing.

3. Maintains current CPR certification.

4. Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order.

5. Minimum of two years' experience in public health or home care nursing is preferred.

6. Self-directed and able to work with minimal supervision.

7. Demonstrates excellent observation, problem-solving, verbal, and written communication skills; nursing skills per competency checklist.

8. Shows ability to organize and prioritize workload independently.

9. Management experience not required. Responsible for supervising home health aides.

10. Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.

Company Description

Our Home Health Care Services are covered by Medicare, Medi-Cal, and most private insurance plans. Certain medical and eligibility requirements need to be met. Our staff will work with the patient's insurance companies to coordinate care and benefits.

Our services are available to patients who need intermittent, skilled nursing care at the convenience of their own private home, residential care facilities, independent care facilities, or in an Assisted Living. Our Home Health Care Team works closely with the patient's physician to provide the utmost care possible to our patients.
