Key Responsibilities- Collaborate with the Interdisciplinary Team to deliver quality patient care under the direction of a licensed nurse.
- Deliver care with consideration to age-specific needs and care.
- Contribute to overall unit functions and maintain a safe and clean environment.
- Assist with admissions, discharges, and transfers, ensuring complete room setup and inventory of patient belongings.
- Provide patient care activities, including hygiene, positioning, and documentation of daily care.
- Participate in the completion of plan of care to meet established goals as delegated.
- Communicate effectively with patients, families, and healthcare team members.
- Current certification as a Nursing Assistant for long-term care and skilled nursing required.
- 1 year of recent experience in a geriatric setting or acute care hospital preferred.
- Effective interpersonal skills and ability to perform all baseline resident/patient care skills under guidance of licensed nurse.
- Fluent knowledge (written and spoken) of English.
