Join Our Team as a Geriatric Care Specialist

We are seeking a highly skilled and compassionate Geriatric Care Specialist to join our team at WellBe Senior Medical. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to our elderly patients in the comfort of their own homes.

Key Responsibilities:
  • Deliver primary care in the home-based setting for patients with chronic and acute illnesses and injuries
  • Conduct initial visits and assessments to onboard eligible patients onto our care model
  • Collaborate with our broader care team to ensure comprehensive medical records and timely documentation
  • Partner with patients, families, and caregivers to provide educational resources and support
  • Facilitate staff, patient, and family decision-making through effective communication and collaboration
Requirements:
  • Valid driver's license and access to a car
  • Previous experience in home care, primary care, hospice, palliative care, or geriatrics
  • Proficiency in Electronic Health Records, Scheduling Software, and Ring Central applications
  • Strong clinical skills and ability to diagnose and treat ongoing, chronic, and urgent medical/psychological conditions
