Responsibilities- Assist the Patient Care Manager in coordinating a team of staff to provide quality care to home health patients.
- Prepare patient lists and necessary documents for weekly case conferences.
- Provide direct patient care as necessary, in accordance with scope of practice, plan of care, and physician orders.
- Assist in clinician documentation review/audits to ensure complete and accurate submission of all documentation by field staff.
- Current CPR certification required.
- Current LPN licensure in state of practice.
- Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.
$65,000 - $80,000 per year, depending on experience.
