Position Overview

This role involves evaluating physician prescriptions to guarantee that the appropriate medications are administered to patients of all ages, from newborns to seniors, ensuring that the correct drug and dosage are prescribed, dispensed, and monitored effectively.

Key Responsibilities:
  • Prescription Review: Assesses all prescriptions for accuracy and suitability, dispensing the most appropriate medications for each patient.
  • Intravenous Preparation: Oversees the preparation of intravenous solutions and other parenteral products.
  • Pharmacy Operations: Manages pharmacy workflow and provides leadership during shifts, ensuring effective communication between shifts and addressing any issues that arise.
  • Collaborative Care: Engages in patient care discussions with nursing staff, physicians, dietitians, social workers, and other healthcare professionals to support a multidisciplinary approach to patient care.
Qualifications:

Certifications:
CSL PHARM – Controlled Substance
PHARM – Pharmacist

Education:
Bachelor's Degree
Completion of a 6-year Pharm D. degree is preferred.

Additional Responsibilities:

Provides and documents pharmaceutical information to healthcare professionals and patients. Aids in maintaining the hospital formulary by addressing non-formulary orders and assists with pharmacy inventory management. Recommends antibiotic therapies, advises on medication transitions, and educates on duplicate therapies.

Involves in the education of patients, students, and other healthcare professionals through discharge counseling, in-services, and hands-on training. Participates in the onboarding of new pharmacists and pharmacy interns, contributing to the development of pharmacy policies and procedures.

Calculates dosages for medications managed by the kinetics service, adhering to established policies and procedures. Oversees the dosing of medications eliminated by the kidneys and Total Parenteral Nutrition (TPN). Prepares IV medications, including TPNs and narcotic infusions, using aseptic techniques and following safety protocols.

Experience and Skills:

Basic computer skills are necessary for entering prescription data. Strong organizational skills are required to manage multiple tasks effectively. Excellent interpersonal skills are essential for clear communication with individuals and groups at all levels.
