Salary: $28.95 per hour (Hawaii residents)

About the Job:

We're seeking an experienced Registered Nurse to join our team as a Healthcare Coordinator. In this role, you'll work in the field and coordinate the care needs of patients in the local community. You'll be responsible for assessing, planning, and implementing individualized care strategies that are directed toward the most appropriate level of care.

Responsibilities:

  • Coordinate care plans throughout the continuum of care as a single point of contact
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Manage patient care needs and advocate for patients and families as needed
  • Communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously

Requirements:

  • Current, unrestricted RN license in the state of Hawaii
  • 2+ years of clinical experience
  • Experience working directly or collaborating services for long-term care, home health, hospice, public health, or assisted living
  • Intermediate level of proficiency with MS Word, Excel, and Outlook
